Welcome to Grandma Swan Smith Vocational Rehab Program
These Terms and Conditions govern your participation in our vocational rehabilitation services. By enrolling in or participating in the Program, you agree to abide by these terms.
1. Program Overview
The Grandma Swan Smith Vocational Rehab Program is designed to support individuals facing employment barriers through career counseling, job placement, skills training, and other vocational support services.
To participate in our program, you must:
Be at least 18 years of age (or have a guardian’s consent if younger).
Be legally eligible to work in the U.S.Complete an intake assessment and be accepted into the program.
Follow a customized Individual Vocational Plan (IVP) developed by our staff.
3. Services Provided
Participants may receive some or all of the following:
Food Truck Business
General Construction
Culinary Arts
Horticulture
Auto Mechanics
Diesel Mechanics
Aquatics
Electrical & Plumbing
HVAC
Services are based on need, availability, and your vocational goals.
4. Participant Responsibilities
You agree to:
Attend scheduled appointments and training sessions.
Communicate respectfully and professionally with staff.
Notify us in advance of any cancellations.Follow all program rules and safety guidelines.
Make active efforts toward achieving employment and independence.
5. Code of Conduct
We maintain a respectful, inclusive environment. The following behaviors are strictly prohibited:
Discrimination or harassment of any kindUse of drugs or alcohol on program premises
Violence or threats of violenceTheft or damage to property
Violation may result in suspension or removal from the program.
6. Confidentiality and Data Use
We respect your privacy and will protect your personal data in accordance with applicable laws.
Your information will only be shared with potential employers, partner organizations, or agencies with your consent or as required by law.
7. Termination and Suspension
We reserve the right to suspend or terminate participation if:
You violate the Code of Conduct.
You fail to engage meaningfully with the Program.
You provide false or misleading information during enrollment.
8. Disclaimer of Warranties
We make no guarantees regarding job placement, income, or specific outcomes. All services are provided “as-is” and subject to availability.
9. Limitation of Liability
Grandma Swan Smith Vocational Rehab Program is not liable for any loss, injury, or damage resulting from your participation in the Program, except in cases of gross negligence or willful misconduct.
10. Governing Law
These terms are governed by the laws of Salt Lake City, Utah without regard to conflict of law principles.
11. Amendments
We may update these Terms and Conditions at any time. Participants will be notified of changes in writing, and continued participation will constitute acceptance of the new terms.
